Trilogy Care – Registered Aged Care Provider

Trilogy Care is the registered and approved aged care provider. They:

Hold government approval under My Aged Care
Manage Home Care Package funding and budgets
Complete intake and care coordination
Ensure compliance with Aged Care Quality Standards

Desired Supports – Service Delivery Partner

Desired Supports works in collaboration with Trilogy Care. We:

Deliver your in-home support services
Provide trained and consistent support workers
Follow your approved care plan
Communicate regularly with Trilogy Care about your support needs

This partnership allows you to receive high-quality care without confusion or duplication.

Services Delivered by Desired Supports

We deliver practical, everyday support services, including:

Domestic assistance (cleaning, laundry, meal preparation)

Transport assistance for appointments and outings

How to Access Aged Care Support (My Aged Care Process)

If you are new to aged care, the process starts with My Aged Care, the Australian Government’s entry point for aged care services.

STEP 1: Contact My Aged Care

You or a family member can:

Call My Aged Care, or
Complete an online assessment request

This initial step helps determine your support needs.

Step 2: Aged Care Assessment

Depending on your situation:

A Regional Assessment Service (RAS) may assess you for entry-level support, or
An ACAT/ACAS assessment may be completed for a Home Care Package

Step 3: Approval for a Home Care Package

If approved, you will receive:

An approval letter
The level of Home Care Package you are eligible for
A referral code

Step 4: Choose Trilogy Care as Your Provider

Once approved, you can choose Trilogy Care as your Home Care Package provider.
Trilogy Care will:

Complete intake
Develop your care plan
Confirm your funding and services

Step 5: Start Services with Desired Supports

After your care plan is in place:

Desired Supports begins delivering your in-home support services
Services can be reviewed and adjusted as your needs change

We work closely with Trilogy Care to ensure smooth and timely service commencement.

Already with Trilogy Care?

If you are already a client of Trilogy Care and:

Are unhappy with your current service provider, or
Would like to change who delivers your day-to-day support

You can choose Desired Supports as your service delivery provider.

In most cases, we can assist you to transition quickly and smoothly, with minimal disruption to your care.

Entry-Level Support (CHSP)

Some older Australians may be eligible for entry-level support under the Commonwealth Home Support Programme (CHSP).

Desired Supports does not directly provide CHSP-funded services. However, we can:

Who manages my Home Care Package and funding?

Your Home Care Package, funding, and compliance are managed by Trilogy Care. Desired Supports delivers the day-to-day support services under your approved care plan.

I am with Trilogy Care but not happy with my current service provider. Can you help?

Yes. If you are a Trilogy Care client and would like to change your service delivery provider, we can help you transition to Desired Supports quickly and with minimal disruption.

How do I start the My Aged Care process?

You can start by contacting My Aged Care by phone or online. Once assessed and approved for a Home Care Package, you can choose Trilogy Care as your provider and then select Desired Supports for service delivery.

What services can Desired Supports provide?

We provide personal care, domestic assistance, social support, transport assistance, respite, companionship, and help with daily living activities.

Do you provide CHSP services?

Desired Supports does not directly deliver CHSP-funded services. We can, however, guide you through the My Aged Care process and help you understand your options.

What if my care needs change?

Your services can be reviewed at any time. Trilogy Care can arrange reassessment through My Aged Care if your needs increase.
