Support Coordination is funded under the NDIS category “Assistance in Coordinating or Managing Life Stages, Transitions and Supports.” We provide SC up to Level 2, including Support Connection and Coordination of Supports. SC helps participants organise supports, connect with providers, manage plan transactions, problem-solve, and build skills to independently manage their plan.
Yes. A service agreement sets out your rights, responsibilities, and the supports we provide. It covers service hours, included supports, pricing, and participant/provider responsibilities. All participants are encouraged to review and sign their service agreement before supports begin.
We request at least 24 hours notice for cancellations or reschedules. Late cancellations may incur a fee depending on your service agreement. Emergency situations are reviewed on a case-by-case basis.
Yes. Support includes meal preparation, household tasks, personal care, community participation, and life skills development.
